A record of an occurence when the Patient experienced an unplanned disruption in personal attendant care.

A record of an observation of the predominant urinary device used by the patient to drain the bladder.

A record of Body Mass Index (BMI) of the Patient enrolled in the Spinal Cord Injury and Disorders program. The BMI is derived from the height and weight of the patient. As the patient's height changes slowly over time, this record is primarily showing changes in the patient's weight. When using Detailed Clinical Models (DCMs) for the Height, Weight, and BMI, a number of the properties that this class inherits from its supertype are duplicative and can be derived from the DCMs.

This class contains the cause(s) of the Patient's spinal cord condition as determined by the clinician. Both traumatic and non-traumatic causes may be entered. [FileMan 154,5.01->154.004].

Identifies a VA Medical Center at which the patient has received care.

A record of Patient Education topics provided to the patient under the auspices of the Spinal Cord Injury and Disorders program. The VA SCIDO Patient_Education table contains a single record for each date upon which a change is recorded, each record indicating whether the patient received any of sixteen possible education topics, and the dates upon which they were received. Here we have modeled it as a single instance per education offered.

The Medical Needs/Function Modifiers (MNFM) is a subset of the Inpatient Rehabilitation Facility – Patient Assessment Instrument (IRF-PAI). It collects information pertaining to swallowing status, clinical signs of dehydration, bladder frequency of accidents in the past seven days, and bowel frequency of accidents in the past seven days.

This class contains summary score information from the last occurrence of the administration of the American Spinal Injury Association Standard Neurological Classification of Spinal Cord Injury (ASIA) Assessment Instrument to the Patient. This information is stored separately in the VA SCIDO application for ease of access - all of these properties can be calculated by retreiving the score and subscore information from the latest instance of the ASIA administration events for the Patient. Therefore, all the properties of this class are marked as derived, and, other than using this class to point to the instrument, might not be implemented at all.

This class points to an administration of the Alcohol Use Disorders Identification Test (AUDIT) Assessment Instrument to the Patient. The VA's SCIDO system contains a number of Assessment Events which could conceivably be used in other systems (such as a Behavioral Health system). This class simply serves to link the AUDIT assessments in SCIDO to the more general AUDIT Assessments modeled in the Assessments package. Logically, neither this class nor the parent SCI_Assessment should exist, but rather the SpinalCordRegistration class should point directly to the relevant AUDIT assessments.

This class points to an administration of the CAGE Assessment Instrument to the Patient. The VA's SCIDO system contains a number of Assessment Events which could conceivably be used in other systems (such as a Behavioral Health system). This class simply serves to link the CAGE assessments in SCIDO to the more general CAGE Assessments modeled in the Assessments package. Logically, neither this class nor the parent SCI_Assessment should exist, but rather the SpinalCordRegistration class should point directly to the relevant CAGE assessments.

This class points to an administration of the Craig Handicap Assessment and Reporting Technique Short Form (CHART-SF) Assessment Instrument to the Patient. The VA's SCIDO system contains a number of Assessment Events which could conceivably be used in other systems (such as a Behavioral Health system). This class simply serves to link the CHART-SF assessments in SCIDO to the more general CHART-SF Assessments modeled in the Assessments package. Logically, neither this class nor the parent SCI_Assessment should exist, but rather the SpinalCordRegistration class should point directly to the relevant CHART-SF assessments.

This class points to an administration of the Check Your Health (CYH) Assessment Instrument to the Patient. The VA's SCIDO system contains a number of Assessment Events which could conceivably be used in other systems (such as a Behavioral Health system). This class simply serves to link the CYH assessments in SCIDO to the more general CYH Assessments modeled in the Assessments package. Logically, neither this class nor the parent SCI_Assessment should exist, but rather the SpinalCordRegistration class should point directly to the relevant CYH assessments.

This class points to an administration of the Short Form version of the McGill Pain Questionnaire (SF-MPQ) Assessment Instrument to the Patient. The VA's SCIDO system contains a number of Assessment Events which could conceivably be used in other systems (such as a Behavioral Health system). This class simply serves to link the SF-MPQ assessments in SCIDO to the more general SF-MPQ Assessments modeled in the Assessments package. Logically, neither this class nor the parent SCI_Assessment should exist, but rather the SpinalCordRegistration class should point directly to the relevant SF-MPQ assessments.

This class represents an "Assessment" of the Patient, either using a standardized Assessment Instrument (such as the American Spinal Injury Association Standard Neurological Classification of Spinal Cord Injury (ASIA)), or a set of observations made about the Patient by a SCIDO clinician. In the VA SCIDO system, each Assessment is stored in a separate table, with a number of fields common to them (such as date created and date modified). This class contains those properties that are common to many if not all of the assessment tables. It is noted that when the assessment uses a standardized Assessment Instrument, the Assessment Event class (in the Assessment package) contains many of these same properties, so these properties are marked as derived.

When a Spinal Cord Injury and Disorders Patient begins either an inpatient rehabilitation or an outpatient rehabilitation, this is the target values that were set with the Patient for what their CHART-SF scores are desired to be by the end of their rehabilitation.

An Episode of Care can be described as health care services provided during a certain period of time, focused on a particular goal. In the SCIDO application, assessments within an episode of care will have the same care type and care start date. Episodes provide a useful basis for analyzing quality of care and utilization patterns.

This class contains information about a Patient who has been enrolled in the Spinal Cord Injury and Disorders registry. It contains summary observations about the Patient's condition, and of the facilities treating the Patient, and of Providers responsible for various aspects of the patient's care. This class is equivalent to FileMan File 154.
